Two complaints were received alleging concerns with fall prevention. Two of two complaints were substantiated. Tenant 1 experienced at least three documented falls since admission and continued to be at risk due to inconsistent staff assistance calls.
Findings
Two complaints were substantiated, and one deficiency was identified related to the failure to update the risk agreement when Tenant 1's condition and service needs changed, increasing fall risk. Tenant 1 had multiple falls and a tendency to self-transfer without calling for assistance, but the risk agreement was not updated accordingly.

Complaint Details
The complaint alleged tenants did not receive timely assistance with toileting and transferring, and concerns about staff ignoring call lights, disrespectful behavior, and inappropriate discharge notices were investigated and substantiated.
Findings
The complaint was substantiated. Deficiencies were found related to insufficient assistance with activities of daily living, inadequate health monitoring and medication management, poor staff supervision, long call light response times, disrespectful staff behavior, and an inappropriate involuntary discharge notice. Several tenants experienced delayed care and distress, and management failed to adequately investigate or address these issues.

Tenant 1 was not treated with courtesy, respect, and dignity by Caregiver L, who swore at Tenant 1 and admonished him/her for using the call pendant.
