Findings
Across two inspections, multiple deficiencies were found including failure to ensure resident safety related to wandering, inadequate alarm systems, use of restraints, poor cleaning practices, unsafe storage of hazardous materials, and improper use of resident sleeping areas.

Policies and procedures are established, documented, and implemented that ensure the safety of a resident who may wander; Based on documentation review, record review, and interview, the manager failed to ensure policies and procedures to ensure safety of wandering residents. |
