Visit Reason
The Office of the State Fire Marshal conducted a fire safety inspection at the Pioneer Place Alzheimer Residence of Tacoma facility on 04/24/2023.
Findings
The inspection found multiple violations related to fire hydrant clearance, working space clearance, owner's responsibility for fire-resistance-rated construction inventory, duct and air transfer openings maintenance, smoke detector sensitivity testing, fire door inspection and testing, and fire drills documentation. The facility was unable to provide required documentation for several items and was disapproved.

Three feet of clearance required around fire hydrant located between both buildings.
Working space clearance must be maintained in the fire alarm panel closet.
Facility shall provide an inventory of all fire-resistance-rated construction in the building.
Unable to provide documentation showing that fire/smoke damper inspection and testing has been performed in the past 5 years.
Unable to provide documentation showing that smoke detectors sensitivity testing has been performed in the last 5 years.
Facility was unable to provide documentation showing that an annual fire door inspection has been completed in the past 12 months.
Facility cannot provide documentation for the completion of twelve planned and unannounced fire drills in the previous 12 months; multiple quarters missing for various shifts.
