Visit Reason
The Minnesota Department of Health conducted an initial survey to evaluate and assess compliance with state licensing statutes for Prestigious Care Homes LLC.
Findings
The facility was found to be in substantial compliance but had multiple deficiencies including food service violations, missing grievance procedure postings, incomplete emergency preparedness plan, fire safety issues, contract deficiencies, incomplete nursing reassessments, medication storage and labeling issues, and unsafe oxygen storage.
