Visit Reason
The Minnesota Department of Health conducted a full survey to evaluate and assess compliance with state licensing statutes for the assisted living facility with dementia care license.
Findings
The survey identified multiple violations including failure to provide compliant egress windows and door locking mechanisms, lack of interconnected smoke alarms, incomplete fire safety and evacuation plans, inadequate staff training on fire safety, and improper use of extension cords. The facility had implemented a temporary fire watch and planned installation of sprinkler and fire alarm systems.
