Through telephone interviews, video‑touring, resident and staff interviews, and a review of repair invoices and pest‑control records, the investigator found that roof leaks in at least one resident’s room persisted despite prior repairs, and that the facility’s cleaning protocols allowed trash to remain in hallways overnight—both conditions that posed health and safety risks. These deficiencies were deemed substantiated, and the department issued corrective action plans with a 29‑April 2021 deadline to complete roof repairs and revise trash‑handling procedures. The roach allegation, however, was not supported by the evidence gathered; most residents and staff reported no visible infestation, and the facility’s monthly pest‑control service appeared adequate, leading to an unsubstantiated finding.

A separate complaint, filed on September 9 2019, alleged that a resident had been molested while in care. Evaluator Kruz Long reviewed staff schedules, resident rosters, pest‑control records, and conducted interviews during an on‑site visit in September 2019, followed by a telephonic review in April 2021. The investigation concluded that the allegation could not be substantiated, and the facility was not found in violation of licensing regulations regarding resident safety.
