The second visit, on 4 November 2021, revealed that the facility had admitted and retained a resident who required total assistance with all activities of daily living—a prohibited health condition under Section 87615(a)(5) of Title 22. The facility received a citation and a plan of correction with a due date of 8 November 2021.
A 25 June 2024 complaint investigation substantiated allegations that staff mismanaged residents’ medications. Review of the Centrally Stored Medication and Destruction Record and the Medication Administration Record showed inaccurate documentation for at least one resident. The investigation cited CCR 87465(h)(6)(C)(D), requiring the licensee to implement daily accountability checks, train medication technicians, and provide evidence of completed training by the stated deadline.
These findings illustrate a pattern of challenges in medication documentation and resident eligibility that surfaced after the facility’s initial compliance period. While the 2021 visits found no issues during the pre‑licensing stage, subsequent inspections have highlighted recurring deficiencies related to Title 22 regulations.
