Testing confirmed that the devices responded to activation, and staff reported no failures. Because no evidence of malfunction was found, the allegation was deemed unsubstantiated and no corrective action was required.
The September 29, 2022 investigation centered on a former resident’s request for medical records. The resident had emailed the facility on September 9, 2022, asking for copies of medication logs and physician communications. The administrator explained that the facility requires a completed release‑of‑records form before providing documentation. The resident had not returned the form, and the agency determined that the facility’s policy contradicted state regulations that mandate prompt access to all resident records. The allegation was substantiated, and a deficiency (CCR 87468.2(a)(19)) was cited with a plan of correction deadline of October 3, 2022.
In the November 29, 2022 inspection, the complaint alleged that staff failed to safeguard a resident’s personal belongings and that a resident had misplaced approximately $200. Interviews with the resident, other residents, and staff revealed no evidence of missing items or staff negligence. Records indicated that the resident managed personal finances independently and had not requested a locked storage area. Consequently, the allegation was again found unsubstantiated.
Across these inspections, the agency consistently verified operational systems and compliance with record‑keeping requirements. While the facility passed the safety and signal‑system checks, it failed to meet the statutory duty to provide prompt access to former residents’ records. The substantiated finding prompted a formal corrective action plan, reflecting a pattern of regulatory attention to documentation access rather than broader safety or security deficiencies.
