Visit Reason
The Minnesota Department of Health conducted an initial survey on October 24, 2024, to assess compliance with state licensing statutes for Remmy's Tender Care LLC Limited Liability Company, an assisted living facility.
Findings
The licensee was found to be in substantial compliance but had several deficiencies including failure to ensure food was prepared and served according to the Minnesota Food Code, lack of a comprehensive tuberculosis prevention and control program, incomplete emergency preparedness plan, physical environment issues, inadequate fire safety and evacuation plans and drills, incomplete background study affiliations for employees, and incomplete resident nursing reassessment documentation.
