Visit Reason
The inspection was conducted as a biennial survey to assess compliance with the Regulations Governing the Licensing and Functioning of Assisted Housing Programs for a PNMI Level IV Residential Care Facility.
Findings
The facility was found non-compliant in multiple areas including medication order review timeliness after hospital discharge, medication storage practices, staff certification training, facility maintenance, safety requirements regarding extension cords, and infection prevention and control practices related to MRSA management.

Visit Reason
The inspection was conducted as a complaint investigation to assess compliance with regulations governing the licensing and functioning of assisted housing programs at Richmond Eldercare.
Findings
The facility was found not in substantial compliance with multiple regulatory requirements including failure to post menus conspicuously, poor sanitation and maintenance issues throughout the facility, water temperature exceeding the allowed maximum, and lack of window coverings in a resident bedroom. Several deficiencies were repeat findings from a prior inspection dated 4/4/2023.
