Home Information River‘s Edge

Inspection Report Summary for River‘s Edge

On September 20, 2021 the Department conducted an unannounced annual inspection of Chateau at River’s Edge, a 740‑type assisted‑living facility with 143 beds. The evaluator reported the building, common areas, resident rooms, bathrooms, kitchen, food storage, medication vault, and fire‑safety equipment were all in good repair, odor‑free, and compliant with Title 22 regulations. The facility had recently completed a COVID‑19 mitigation plan, was conducting routine symptom screening, and maintained adequate food supplies and hand‑hygiene protocols. No deficiencies were cited and the administrator, Michael Talani, received the report without further action.

The same day a separate case‑management visit addressed an incident of a missing coin purse (R1) that had occurred on September 7. An incident report and police record were reviewed, the resident was interviewed, and the administrator confirmed that proper reporting and retraining procedures were followed. Again, the evaluator found no deficiencies and closed the matter. Subsequent case‑management visits on October 7 and December 6 addressed a series of thefts totaling more than $1,400 from residents’ apartments and prescription sunglasses. In each instance the executive director reported full cooperation with law enforcement, conducted internal investigations, retrained staff on theft prevention and mandated reporting, and implemented a policy for residents to list valuables upon admission. No new deficiencies were identified.

A later visit on May 19, 2022 focused on a caregiver‑related fall that occurred when a resident (R1) was trapped in a recliner because the caregiver had raised the footrest. Video evidence demonstrated the improper maneuver and the resulting fall. The evaluator cited a deficiency under Title 22, issued a deficiency notice with a correction plan due by May 23, and provided the facility with appeal rights. No other violations were found during this inspection.

Overall, Chateau at River’s Edge demonstrates consistent compliance with environmental and safety standards. The recurring theme involves resident thefts, largely linked to staff from a resource agency, prompting enhanced training and policies. The sole documented deficiency in May 2022 highlights the need for stricter caregiver procedures to prevent resident falls. The facility’s prompt investigative responses and corrective actions have maintained its licensing status and addressed the identified concerns.
