Findings
The facility was found to be in non-compliance with multiple rules including improper storage of linens posing cross contamination risks, unlabeled food items in the memory care refrigerator, and unsecured hazardous paint bottles in the memory care unit. Violations were established for laundry and linen requirements, kitchen and dietary safety, and general maintenance and storage.
