Deficiencies were cited related to failure to maintain fire safety components and mechanical components in a safe and operating condition, including an attic access panel that does not seal tightly and excessive particulate build-up on the Employee Bathroom mechanical exhaust fan grille.

The facility was surveyed for conformance with the applicable portions of the 2005 Rules for Licensing of Adult Care Homes of Seven or More Beds, and applicable portions of the North Carolina Building Code and Minimum Standards and Regulations for Homes for the Aged, as part of a Biennial Construction Survey.

Findings
The survey found multiple deficiencies including excessive combustible storage in certain rooms, unsafe latching hardware on a bathroom door, compromised fire-rated walls and ceilings with holes and penetrations, corridor doors that do not close or latch properly, a malfunctioning warning device for an emergency release switch, and a non-functioning exhaust fan in a bathroom.
