The initial health‑and‑safety inspection in May found no deficiencies, and a later visit on 3 March 2022—an annual compliance check—also reported no violations. These inspections confirmed that the facility maintained adequate infection‑control measures, proper fire safety, and up‑to‑date staff training.
In November 2021 the facility faced a more intensive review. Two separate case‑management inspections uncovered safety and documentation gaps. An unsupervised steak knife in a kitchen, a disinfectant bottle left on a toilet tank, and incomplete or outdated medical assessments for residents with dementia highlighted vulnerabilities in both physical safety and care planning. While no immediate health‑or‑safety deficiencies were cited in one of the November visits, the licensing analysts issued technical violations for inadequate medical assessment procedures and provided technical assistance on hospice care, pressure injuries, and medication guidance. A subsequent inspection on 10 November focused on the plan‑of‑correction issued earlier in the month and found no new deficiencies.
The pattern that emerges is one of a facility generally compliant with core safety and staffing requirements but experiencing intermittent lapses in resident‑care documentation and some food‑service safety protocols. The repeated unannounced inspections and the issuance of a plan of correction indicate that CCLD is actively monitoring Rosewood’s adherence to California Code of Regulations, Title 22. With corrective action taken and no further deficiencies noted in the latest annual review, the facility appears to be moving toward full compliance, although ongoing attention to medical assessment updates and kitchen safety will be essential to maintain regulatory standards.
