Visit Reason
The Minnesota Department of Health conducted a survey to evaluate and assess compliance with state licensing statutes for Sabainah Healthcare Inc, an assisted living facility.
Findings
The licensee was found to be in substantial compliance but had several deficiencies including food preparation not fully compliant with Minnesota Food Code, incomplete employee records, lack of a comprehensive emergency preparedness plan, incomplete fire safety and evacuation plans and drills, inclusion of a liability waiver in resident contracts, and incomplete resident nursing assessments.
