The analyst noted that provider information notices (PINs) were posted only in the medication room, not in a location visible to residents, and that some private rooms lacked paper towels. The facility administrator agreed to post PINs on the bulletin board and to supply towels. A separate deficiency arose when the analyst discovered one staff member on the roster without a required criminal record clearance. The administrator removed the individual from the schedule and committed to verifying clearances for all employees before they begin work. A civil penalty was issued under CCR 87355(e)(1) and the POC was due February 26, 2022.
During the July 5, 2022 visit, the analyst observed an abandoned washing machine that had not been removed despite a prior promise. The presence of the unused machine violated CCR 87303(a), which requires the facility to be clean, safe, sanitary and in good repair at all times. The citation noted the potential health and safety risk to residents and the public. The POC deadline for removal and cleanup was July 7, 2022.
The September 16, 2022 visit identified a lack of a designated substitute administrator on the premises 24 hours per day, violating §1569.618. This deficiency posed a health and safety risk to residents. Additionally, the analyst observed staff wearing masks that did not fully cover the nose, and the facility did not ensure the personal rights of residents for safe, healthful accommodations as required by §87468.1(a)(2). The POCs for both deficiencies were set for September 19, 2022, with the same civil penalty risk if not corrected.
Across all four inspections, recurring themes emerged: inadequate administrative presence, lapses in staff credentialing, and deficiencies in maintaining a clean, safe environment.
