Visit Reason
The Minnesota Department of Health conducted an initial survey to assess compliance with state licensing statutes for an assisted living facility license.
Findings
The licensee was found to be in substantial compliance but had several deficiencies including incomplete employee records, lack of tuberculosis screening documentation, incomplete emergency preparedness plan, non-interconnected smoke alarms, incomplete fire safety and evacuation plan and training, and insufficient dementia care training for staff.
