Visit Reason
The Adult Care Licensure Section conducted an annual and follow-up survey from 11/13/24 to 11/15/24 to assess compliance with regulations including housekeeping, personal care and supervision, health care, medication administration, and controlled substances management.

Findings
The facility failed to maintain cleanliness in a resident's room and bathroom, failed to provide adequate supervision for a resident with smoking behaviors, failed to ensure health care coordination and follow-up for a resident with injuries from falls, failed to administer medications as ordered for two residents including insulin and controlled substances, and failed to maintain accurate controlled substance records.

Resident #1's room and shared bathroom in the special care unit were not maintained in a clean and orderly manner with cigarette burn holes, stains, dirt, and smoke odor present.
Facility failed to provide supervision for Resident #1 who smoked in unauthorized areas and possessed smoking materials, resulting in burn holes in clothing and bathroom floor, posing a fire risk.
Facility failed to ensure health care coordination and follow-up for Resident #4 including failure to coordinate orthopedic referral for broken arm and failure to obtain x-ray for rib injury.
Facility failed to administer medications as ordered for Residents #1 and #2 including missed doses of Lorazepam for Resident #1 and incorrect insulin dosing for Resident #2.
Facility failed to ensure accurate reconciliation of controlled substances for Resident #1 resulting in 30 unaccounted doses of Tramadol.

Visit Reason
The Adult Care Licensure Section conducted an annual survey of Scotland House from April 12, 2022 to April 13, 2022 to assess compliance with adult care home regulations.

Findings
The facility was found deficient in maintaining an accurate and current listing of residents with physician-ordered therapeutic diets, serving therapeutic diets as ordered, and administering medications according to physician orders. Specific issues included inaccurate therapeutic diet listings, failure to serve mechanical soft diets as ordered for Resident #3, and medication administration errors for Residents #1 and #3.
