Inspection Report Summary for Select Senior Care

Select Senior Care LLC (Facility Number 306005505) has undergone four unannounced inspections between February 2022 and December 2024. The first inspection on 02/28/2022, conducted by LPA Albert Marin, found the two‑story facility in good repair: all resident rooms, bathrooms, and common areas met safety and sanitation standards, fire and carbon‑monoxide detectors functioned, and medications and sharps were stored out of resident reach. The facility complied with its COVID‑19 mitigation plan and the 2022 AB 665 requirement for an internet‑enabled device for resident use. No citations were issued, and the report was passed to the licensee for review.

A case‑management visit on 07/08/2022 addressed an incident involving resident R1 who was removed from the facility by his daughter, missed medications, and suffered a skin tear. After reviewing records and interviewing staff, LPA Michelle Reed concluded that no Title 22 regulations were violated. The report was closed without citations. The subsequent annual inspection on 11/01/2022, performed by LPA Edward Tapia, again confirmed the facility’s compliance: all safety devices were operational, food stock met state minimums, and the Emergency Disaster Plan was current. AB 665 and the COVID‑19 plan were re‑validated, and no deficiencies were noted.

The most recent inspection on 12/20/2024, conducted by LPA Samer Haddadin, uncovered new deficiencies. Sharps and knives in a kitchen drawer were left unlocked due to a broken lock, violating CCR 87705(f)(1) on the care of persons with dementia and creating an immediate safety risk. The report also noted the absence of a documented fire‑drill log, a requirement of Title 22 Division 6. A plan of correction was issued with a due date of 12/27/2024, and failure to comply may result in a civil penalty. This shift from a pattern of compliance to identified safety lapses highlights the importance of ongoing vigilance in securing hazardous items and maintaining routine safety documentation.
