The purpose of this visit was to conduct the initial inspection of SEVA Adult Care.

Findings
The facility failed to obtain required medical examination reports within six months prior to admission for 2 sampled participants, lacked physician orders for therapeutic diets for 1 participant, failed to maintain a Medication Assistance Record for 1 participant, and did not store medications securely as required.
