Sherry’s RCFE, a six‑bed residential care facility located at 3996 Wildrose Way in Sacramento, underwent a series of regulatory evaluations by the California Department of Social Services, Community Care Licensing Division (CCLD) during 2022. Initial compliance interviews on April 5 and April 8, 2022, focused on the Administrator, Sherry Ahuja, and her knowledge of Title 22 regulations. The first interview revealed insufficient understanding of the Code, resulting in a rescheduled assessment; the subsequent interview on April 8 confirmed Ahuja’s grasp of operational, staffing, admission, and emergency requirements, allowing the Component II assessment to be deemed successful.
A pre‑licensing inspection on May 18, 2022, conducted by Licensing Program Analyst Kevin Gould, evaluated the physical plant and health‑safety standards. The facility was clean and structurally sound, but one resident bathroom lacked a non‑slip mat and grab bars, preventing full compliance. In late 2022, CCLD shifted focus to case‑management documentation. On October 28, 2022, Gould identified deficiencies in medication administration records (MARs) for three consecutive days, citing a failure to document resident medications timely—a matter of potential risk to resident safety. A Plan of Correction (POC) was issued with a due date of October 31, 2022. By November 17, 2022, a follow‑up POC inspection found the earlier deficiencies resolved; MARs were complete and accurate, and a POC clearance letter was generated. The pattern of early compliance gaps—initial knowledge deficits and later documentation lapses—was systematically addressed through targeted inspections, corrective plans, and follow‑ups, ultimately leading to the facility’s full compliance with Title 22 requirements.
