Findings
Across two inspections conducted in 2025, multiple deficiencies were identified including failure to report a resident's death timely, inadequate staff training and documentation, incomplete personnel records, and missing documentation of medication administration and smoke detector testing.
Complaint Details
The May 14, 2025 inspection was conducted as an on-site investigation of complaints 00130574 and 00130600. The January 31, 2025 inspection included investigation of complaints AZ00205134 and AZ00221098 along with an annual compliance inspection.
