Inspection Report Summary for Sierra Oaks Assisted Living & Memory Care
Sierra Oaks of Redding (Facility Number 455002787) entered the licensing process with a clean pre‑licensing evaluation. During the unannounced visit on April 5 2022, the facility’s administrator, Kristine Boban, was briefed on the licensing requirements and the site was inspected for safety, sanitation, and physical plant readiness. Inspectors observed fully furnished client rooms, functional bathrooms, operating smoke and carbon‑monoxide detectors, and properly stored medications in a locked cabinet. No deficiencies were recorded, and the facility was deemed ready for licensing by the Centralized Application Bureau.
A subsequent complaint investigation on August 2 2022, however, revealed a substantiated financial abuse allegation. The review found that Sierra Oaks had failed to issue a refund within the 30‑day window required by CCR 1569.652(c) after the death of Resident 1 on June 1 2022 and the removal of the resident’s personal property. The administrator had not returned the prorated balance of $5,314.36, a deficiency that posed a potential health and safety risk. A plan of correction was issued with a deadline of August 9 2022, and the facility was required to provide proof of refund to the Department.
In a third investigation on March 20 2023, complaints regarding staff training adequacy and a disrepair of a kitchen refrigerator door were examined. Interviews and documentation reviewed by evaluators Donna Gurriere and Sarah Benson indicated that staff reported receiving the requisite training hours and the allegation of inadequate training lacked sufficient evidence to be substantiated. The refrigerator door issue was noted, but no corrective action was required under the findings. Across the inspections, the facility maintained appropriate safety equipment, hygienic conditions, and a lack of physical hazards, but the financial compliance issue highlighted a gap in administrative procedures that the agency addressed through the plan of correction.
