Silva Board and Care Facility, a six‑bed group home in Santa Rosa, has consistently met the California Department of Social Services’ core requirements over the past two years. In the 06/17/2021 inspection, licensing analysts Victoria Willis and Erik Gonzalez Campos noted that the facility’s infection‑control program was fully operational, with COVID‑19 posters, hand‑washing stations, adequate personal protective equipment, and a 30‑day supply of medication. Staff and residents were 100 % vaccinated, and the site had no deficiencies cited.

The 05/20/2022 visit uncovered a single non‑compliance issue: a live‑in staff member was sleeping in a hallway closet, a space not designated as a bedroom per fire and California Code of Regulations §87307(a). The LPA required removal of the mattress and documentation that the closet would not be used for sleeping. The report also reiterated that staff had completed PPE training but had not been N95 fit‑tested. No other deficiencies were noted, and the facility was asked to submit updated administrative documentation by late May.

During the 2023 inspections, the facility again demonstrated compliance. A January case‑management visit followed up on a resident fall; the review found no deficiencies. The April 27 annual inspection confirmed that the facility maintained safe temperatures, adequate food storage, working fire safety equipment, and that the facility’s administrative certificate was expired but backed by two other licensed administrators. The evaluators requested that updated liability insurance, personnel summaries, and the infection‑control plan be submitted within a month, though no new deficiencies were cited. Overall, the reports show a pattern of solid infection‑control practices, timely PPE provisioning, and a single, quickly corrected staff‑sleeping‑area issue, with no repeat infractions.
