St. Paul’s Towers, a 741‑type facility in Oakland, has consistently met California Department of Social Services licensing standards during a series of unannounced inspections from September 2021 through late 2022. The initial visit in September 2021 was an infection‑control assessment that confirmed the presence of a single universal screening point, proper PPE use, adequate food and PPE supplies, and documented mitigation, disaster, and screening records. No deficiencies were recorded, and the licensing analyst received a copy of the completed report.
Subsequent inspections focused on case‑management issues. A visit in June 2022 followed a resident’s self‑reported death on May 25, 2022, during which the analyst collected the deceased resident’s documents and confirmed that the resident had been healthy and independent prior to passing. Again, no deficiencies were cited. A series of visits from July through November 2022 examined the integration of 15 residents transferred from Grand Lake Gardens. During each visit the analyst reviewed staff schedules, resident rosters, and interviewed a sample of the new residents. All residents reported feeling safe and that their basic needs—food, paper, and PPE—were met, and the facility’s staffing remained stable. One resident noted affordability concerns, prompting a follow‑up with the transferring community. None of the inspections uncovered imminent health or safety concerns.
Across all visits, St. Paul’s Towers consistently demonstrated robust infection‑control practices, maintained necessary supplies, and kept accurate documentation for screening and emergency planning. The repeated absence of deficiencies, coupled with the regular monitoring of resident transfers and a single unremarkable resident death, indicates a high level of regulatory compliance and a stable environment for residents. These findings suggest that the facility is operating within the expectations of the California Department of Social Services and that any potential risks identified during inspections have been appropriately addressed.
