Inspection Report Summary for Summerfield Memory Care of Roseville
Somerford Place‑Roseville (now operating as Summerfield of Roseville) is a 740‑type residential care facility located at 110 Sterling Court in Roseville, California. The California Department of Social Services (CCLD) conducted a series of unannounced inspections beginning in 2021. Both the 06 / 02 / 2021 and the 02 / 15 / 2022 visits focused on the infection‑control domain. In each instance, the licensing program analyst (LPA) met with the facility’s executive director, toured the premises, and determined the facility was in substantial compliance with all applicable standards. No deficiencies were cited and the reports were completed without incident.
In the required 1‑year inspection on 04 / 12 / 2023, the LPA identified a significant lapse in dementia care. Two of five residents’ physician reports had not been updated for over a year, constituting a failure to meet the annual medical assessment requirement for persons with dementia. The facility was cited under California Code of Regulations §87705(c)(5) and given a plan of correction deadline of 05 / 17 / 2023. Aside from this issue, the inspector noted that fire safety, medication storage, and the overall physical environment met all regulatory expectations; no additional deficiencies were recorded.
Two case‑management incident visits followed in April and May 2023. The April visit investigated a medication‑management issue uncovered during a pharmacy audit; the LPA found no deficiencies, documented staff training on reordering procedures, and noted the facility’s transition to an electronic medication administration system. The May visit addressed an eye‑drop incident reported by a resident’s responsible party. The facility’s internal review confirmed that the drops were being administered and that the pharmacy had recently refilled the supplies. In both cases, the LPAs issued no citations and the incidents were closed after corrective actions were documented.
Overall, the facility demonstrates strong infection‑control and environmental compliance, with the primary recurring concern being the timely completion of medical assessments for residents with dementia. The licensing reports reflect responsive incident management and ongoing training initiatives, indicating a continuing commitment to resident safety and regulatory adherence.
