The investigation included a review of R1’s medical records, facility files, and interviews with staff. X‑ray results and subsequent physician findings indicated a right‑hand fracture, but no documentation or witness testimony linked the injury to staff conduct. The department therefore concluded the allegation was unsubstantiated, meaning there was insufficient evidence to prove or disprove the claim.

Interviews with hospital personnel and the primary care physician revealed no signs of neglect or delayed intervention that could have contributed to the death, leading the department to rule the complaint unfounded. In the pressure‑injury claim, medical records indicated R1 developed a Stage II pressure injury that was treated with topical ointment; there was no evidence that staff neglect caused the injury. That allegation was therefore unsubstantiated.
