The California Department of Social Services Community Care Licensing Division investigated Facility No. 361880786—SUMMERFIELD OF REDLANDS (also recorded as SOMERFORD PLACE‑REDLANDS)—on two separate occasions. In the 2023 visit, evaluator Bernadette Allen examined a complaint that staff had handled Resident #1 (R1) roughly on 10/29/2022. The investigation included a review of R1’s medical records, facility files, and interviews with staff. X‑ray results and subsequent physician findings indicated a right‑hand fracture, but no documentation or witness testimony linked the injury to staff conduct. The department therefore concluded the allegation was unsubstantiated, meaning there was insufficient evidence to prove or disprove the claim.
The earlier 2021 investigation, conducted by evaluator Stephanie Williams, addressed two distinct allegations: a “questionable death” of R1 on 6/20/2020 and the development of pressure injuries while R1 was in care. Records showed R1 was admitted with sepsis secondary to pneumonia, placed on palliative care, and died of sepsis and healthcare‑associated pneumonia. Interviews with hospital personnel and the primary care physician revealed no signs of neglect or delayed intervention that could have contributed to the death, leading the department to rule the complaint unfounded. In the pressure‑injury claim, medical records indicated R1 developed a Stage II pressure injury that was treated with topical ointment; there was no evidence that staff neglect caused the injury. That allegation was therefore unsubstantiated.
Across both investigations, the facility’s administrators—Danica Turner (Administrator) and, in the 2023 visit, Hedi Charette (Administrator)—were met, and findings were communicated via exit interviews or telephone due to COVID‑19 restrictions. The pattern emerging from these reports is a consistent lack of documented evidence linking staff actions to the adverse outcomes alleged by complainants, resulting in a series of unsubstantiated or unfounded determinations.
