Summerfield Senior Living Facility (Facility Number 515002613) in Yuba City, California, has been the subject of several unannounced inspections by the Department of Social Services’ Community Care Licensing Division. The agency’s evaluators have examined a range of complaints that touch on resident care, staff conduct, and compliance with health regulations.
In March 2021, Evaluator Misty Valencia conducted an unannounced investigation in response to allegations of neglect, lack of care, and personal rights violations. Interviews with all seven staff members and all five residents found no evidence that residents’ needs were unmet; the complaints were therefore deemed unsubstantiated. However, a later narrative dated 12/23/2021 documented a separate, substantiated finding: staff at a Christmas party on 12/18/2021 failed to wear masks or maintain social distancing, contrary to California Code of Regulations Title 22. The facility was cited for a personal‑rights deficiency (CCR 80072(a)(2)) and was required to provide staff training on mask policy by 3/19/2021.
A May 2024 inspection addressed a complaint that staff did not prevent one resident from inappropriately touching another. Interviews with the resident, staff, and the resident’s medical team revealed no evidence that staff failed to follow protocol, and the resident reported feeling safe. The allegation was found unfounded, and no deficiencies were cited.
In May 2025, a new complaint alleged that a resident sustained multiple injuries from falls, that staff failed to address the falls, and that the resident was left soiled. The evaluator reviewed resident records and care plans, noting that the resident was listed for frequent checks and that staff regularly monitored residents. Because the evidence did not establish a preponderance of proof that the injuries resulted from neglect or improper care, the finding was unsubstantiated.
Across these investigations, the majority of complaints—both in 2024 and 2025—were resolved without citation, and the 2021 complaint about personal‑rights violations remains the only substantiated finding. The pattern suggests that while the facility generally meets regulatory requirements, lapses in compliance with health and safety protocols (as in the mask violation) can occur. The agency has required corrective actions where deficiencies were identified, and subsequent visits have not revealed repeated violations.
