On September 19, 2024 the California Department of Social Services Community Care Licensing Division conducted an unannounced inspection of Sundial Assisted Living, a 740‑bed facility located at 395 Hilltop Drive in Redding. The visit, led by Licensing Program Analyst Farhaan Sarangi, involved reviewing resident and facility records, interviewing current staff, a former employee, and residents, and observing daily operations. Administrator Elizabeth Amlin welcomed the evaluator and granted full access.

The investigation addressed nine complaints filed on August 22, 2024. Allegations ranged from residents not being changed promptly and receiving insufficient hydration, to concerns about finger‑stick glucose checks, medication charting discrepancies, missing narcotics, failure to monitor oxygen equipment, non‑refill of prescriptions, and a staff member testing COVID‑positive. In each case, the evaluator obtained inconsistent statements and could not corroborate the claims. Documentation and direct observation of medication administration records, resident care logs, and facility practices revealed no evidence supporting the allegations, leading the report to classify all nine complaints as unsubstantiated.

Despite the unsubstantiated findings, the inspection uncovered two regulatory deficiencies. First, the facility failed to submit required incident reports to the Community Care Licensing Division within the seven‑day window mandated by CCR 87211(a)(1)(d). Second, the site did not meet the observation standards for residents stipulated in CCR 87466, which require regular assessment of residents’ physical, mental, emotional, and social functioning. Both deficiencies were cited with plan‑of‑correction deadlines—September 20, 2024 for resident observation and September 26, 2024 for incident reporting. Administrator Amlin was informed of these obligations and provided training resources and corrective action guidelines. Failure to address the deficiencies by the due dates could result in civil penalties. The report, signed by the evaluator and the facility representative, will remain publicly available for three years.
