Staff did not promptly notify the resident’s conservator when a potential UTI was identified, and the resident did not receive a urinalysis until more than a month after the initial concern. The facility also admitted the resident to the facility before obtaining the conservator’s written consent and moved the resident to a new room without providing the mandated thirty‑day written notice or the conservator’s approval. These omissions were cited under multiple sections of the California Code of Regulations, requiring the creation of a plan for incidental medical care and a comprehensive plan of operation addressing dementia care, communication with conservators, and documentation of written consents. The plan of correction deadlines were set for August 5, 2021, and August 18, 2021, respectively.
