Inspection Report Summary for Sunrise of Carmichael
During the unannounced inspection on August 4, 2021, the California Department of Social Services (CCLD) evaluated Sunrise Assisted Living of Carmichael, 5451 Fair Oaks Blvd., Carmichael, CA. Evaluator Angela Hood met with Executive Director Davina Barker and reviewed resident records, incident reports, and staff communications. The investigation centered on five complaints: staff’s failure to report a resident’s change in condition, delayed medical attention for a urinary tract infection, an unwitnessed fall that injured a resident, admission of a resident without the responsible party’s consent, and a room move made without consent. The report concluded that all allegations were substantiated except for the fall, where the facility’s response met required care standards, so no deficiency was cited for that incident.
The findings highlighted several compliance gaps. Staff did not promptly notify the resident’s conservator when a potential UTI was identified, and the resident did not receive a urinalysis until more than a month after the initial concern. The facility also admitted the resident to the facility before obtaining the conservator’s written consent and moved the resident to a new room without providing the mandated thirty‑day written notice or the conservator’s approval. These omissions were cited under multiple sections of the California Code of Regulations, requiring the creation of a plan for incidental medical care and a comprehensive plan of operation addressing dementia care, communication with conservators, and documentation of written consents. The plan of correction deadlines were set for August 5, 2021, and August 18, 2021, respectively.
The investigation reinforced the importance of timely medical assessment and clear communication with responsible parties in assisted‑living settings. While the facility’s response to the fall met safety expectations, the documented lapses in medical coordination, consent documentation, and resident‑change notifications represent significant risks to resident health, safety, and personal rights. The cited deficiencies and required corrective actions underscore the necessity for Sunrise Assisted Living of Carmichael to strengthen its policies and training to ensure compliance with state regulations and to safeguard resident welfare.

Frequently Asked Questions
Several government programs offer financial assistance for senior housing in various countries, focusing on the United States for a broad overview:
- Medicaid: Medicaid is a state and federally funded program that can cover the costs of nursing home care for those who meet eligibility criteria, including income and asset limits. Some states also offer Medicaid waivers that help pay for home and community-based services to prevent or delay nursing home placement.
- Medicare: Medicare, primarily a health insurance program for people aged 65 and over, does not cover long-term housing costs. However, it can cover short-term stays in a skilled nursing facility under specific conditions following a hospital stay.
- Section 202 Supportive Housing for the Elderly Program: This program provides housing for low-income seniors. It offers rental assistance and access to supportive services, such as cleaning, cooking, and transportation.
- Low-Income Housing Tax Credit (LIHTC) Properties: While not a direct subsidy, LIHTC encourages developers to create affordable housing. Seniors with low incomes can find reduced-rent apartments through this program.
- State and Local Programs: Many states, counties, and cities offer their own programs to assist seniors with housing costs. These can include property tax relief programs, rental assistance programs, and programs that offer affordable senior housing options.
- Veterans Affairs (VA): The VA offers several programs for veterans, including the Aid and Attendance benefit, which provides monthly payments to veterans who require the aid of another person, or are housebound, to help cover the cost of care in homes, nursing homes, and assisted living facilities.
- Social Security: While Social Security primarily provides retirement income, for many seniors, these benefits are a crucial part of their budget, including housing costs.
