The visit was a Final Recertification Monitoring Evaluation conducted to review the Plan of Correction in response to a Preliminary Recertification Monitoring Evaluation Report for Terrace Park Assisted Living.

Findings
The report found no substantiated regulatory insufficiencies during the certification period. The Plan of Correction was accepted by the Department of Inspections and Appeals. Tenant evaluations and service plans showed some regulatory insufficiencies related to timely and comprehensive assessments.
