Visit Reason
The inspection was a follow-up review conducted on 03/01/2024 to verify that the submitted plan of correction was fully implemented at The Fountains at DuBois Luxury Senior Care, LLC.

Findings
The facility was found to have implemented all required corrections related to carbon monoxide alarm placement, trash management, bathroom ventilation, emergency procedures posting, fire drill completion, and exit signage. Continued compliance and monitoring were mandated.

Carbon monoxide alarm was approximately 8 feet from the gas dryer in the laundry room, not meeting the required minimum distance of 15 feet.
The lids of the dumpster in the rear of the facility did not close due to overflowing trash.
The common bathroom in the lobby area of personal care does not have an operable window or ventilation fan.
The municipality's emergency procedures were not posted in a conspicuous and public place in the home.
A fire drill had not been conducted by a fire safety expert.
No exit sign over the staff break room door in personal care area that leads from the resident hallway to emergency exit B.
