| A. A governing authority shall: 9. Ensure compliance with A.R.S. § 36-411. The governing authority failed to verify the status of a personnel member's fingerprint clearance card as required by A.R.S. § 36-411(C)(2). |
| C. A manager shall ensure that a personnel record for each employee or volunteer: 1.c.ii. Documentation of the individual's education and experience applicable to the individual's job duties. The manager failed to ensure personnel records included documentation of education and experience for four of five caregiver records sampled. |
