Findings
Across four inspections, including complaint investigations and a compliance inspection, the facility had four deficiencies related to personnel record documentation, resident service plans, medication administration, and governing authority compliance. Complaint investigations were conducted on April 17, 2025; August 15, 2024; January 17, 2024; and September 26, 2023. Two complaint inspections found no deficiencies, while two others found deficiencies related to personnel records, service plans, and medication administration.
Deficiencies (4)
Description
A. A governing authority shall: 9. Ensure compliance with A.R.S. § 36-411. The governing authority failed to verify the status of a personnel member's fingerprint clearance card as required by A.R.S. § 36-411(C)(2).
C. A manager shall ensure that a personnel record for each employee or volunteer: 1.c.ii. Documentation of the individual's education and experience applicable to the individual's job duties. The manager failed to ensure personnel records included documentation of education and experience for four of five caregiver records sampled.
A. Except as required in subsection (B), a manager shall ensure that a resident has a written service plan that: 3.c. The amount, type, and frequency of assisted living services being provided to the resident, including medication administration or assistance in the self-administration of medication. The manager failed to ensure service plans included the amount, type, and frequency of services for three of eight residents sampled.
B. If an assisted living facility provides medication administration, a manager shall ensure that: 3.a. Is administered by an individual under direction of a medical practitioner, b. Is administered in compliance with a medication order, and c. Is documented in the resident's medical record. The manager failed to ensure medication was administered in compliance with orders and properly documented for multiple residents.
