The investigation included review of resident records, observation of care and services, and interviews with residents, family members, and staff.

Complaint investigated regarding failure to report incidents of abuse to appropriate state agencies, failure to put interventions in place to help prevent falls, failure to provide services as agreed upon, and failure to ensure proper handwashing during dietary services. No violations were found related to abuse reporting, fall prevention, service provision, or handwashing.

The facility reported incidents of resident to resident abuse as required with no violation found. Interventions to prevent falls were in place and effective. Resident care, meal service, activities, and medication administration were conducted appropriately with no violations. Proper handwashing was observed during dietary services.
