KENSINGTON AT WALNUT CREEK, a 740‑type residential care facility for the elderly located at 1580 Geary Road, Walnut Creek, completed a change‑in‑ownership application in June 2023. The department’s Office Evaluation on 6 14 2023 confirmed that Administrator Deborah Bradley and the applicant had met all licensing knowledge requirements and had signed the required acknowledgment. The facility maintained a census of 143 residents, well below its 200‑person capacity.

In late July, a pre‑licensing inspection was conducted unannounced on 7 24 2023. Licensing Program Analysts L. Alexander and L. Hall toured the entire eight‑building campus, noting that all rooms, bathrooms, and common areas complied with fire, safety, and health standards. Two medication rooms remained locked, temperature controls were within acceptable ranges, and all required fire extinguishers and detectors were functional. Ten staff and resident files were reviewed with no deficiencies noted. The facility was deemed ready for licensing, pending final review by the Central Applications Unit.

Follow‑up case‑management visits in October 2023 and April 2024 addressed two separate incidents. In October, the LPA gathered documentation after the death of Resident R1 on 10 12 2023, noting that the death certificate and police report would be furnished by the family; no deficiencies were cited. In April, an Unusual Incident Report revealed that a medication error occurred when Staff S1 mistakenly gave another resident’s medication to R1. The incident was reported, the resident was monitored for 72 hours, and the staff received immediate retraining. The LPA documented the deficiency and required corrective action under Title 22, setting a due date of 4 25 2024.
