Visit Reason
The inspection was a Construction Section Biennial Survey conducted to assess conformance with the 2005 Rules for Licensing of Adult Care Homes of Seven or More Beds and applicable portions of the 2012 Edition of the North Carolina Building Code, Institutional Occupancy.
Findings
Multiple deficiencies were cited including lack of current fire and building safety inspection reports, obstructions in corridors, unclean and unrepaired ceilings, failure to maintain fire safety and electrical equipment in safe operating condition, inadequate exhaust ventilation in required spaces, and improper maintenance of plumbing and fire safety components.

Facility did not have fire and building safety inspection reports maintained in the home and available for review.
Failure to maintain corridors free of equipment and obstructions, reducing clearance to less than six feet.
Ceilings were not kept clean and in good repair, with water stains and mildew spots observed.
Facility was not maintained in an uncluttered, clean and orderly manner, with combustible materials stored behind dryers.
Not all electrical outlets in wet locations had functioning ground fault interrupters, posing a shock hazard.
Failure to maintain emergency fire alarm system devices and equipment in safe operating condition, including issues with smoke detectors, fire doors, exit signs, and emergency lighting.
Failure to maintain plumbing piping with a minimum 2" air gap, risking contamination of domestic water supply.
Holes or gaps at penetrations through fire resistant rated ceilings could allow fire and smoke to spread beyond the area of origin.
Use of unapproved devices to keep doors open, impeding quick closing and limiting spread of smoke or fire.
Failure to maintain electrical emergency/safety lighting equipment in safe operating condition, including emergency light full of dirty water.
Failure to provide exhaust ventilation in required spaces such as janitor's closet and resident laundry.
