Visit Reason
The Adult Care Licensure Section conducted an annual survey and complaint investigation from 10/11/23 to 10/12/23. The complaint investigation was initiated by the Halifax County Department of Social Services on September 21, 2023.

Complaint investigation was initiated by the Halifax County Department of Social Services on September 21, 2023, related to Resident #1's care and medication administration.
Findings
The facility failed to ensure implementation of physician orders for one sampled resident related to notifying the primary care provider of fingerstick blood sugars outside of parameters and failed to administer medications as ordered, including insulin and topical medication. Additionally, the facility failed to issue a refund within 14 days to one discharged resident.
