Visit Reason
This facility was surveyed for conformance with the 2005 Rules for Licensing of Adult Care Homes of Seven or More Beds and applicable portions of the 2018 Edition of the North Carolina Building Code, Institutional Occupancy during a Construction Section Biennial Survey.

Findings
Deficiencies were cited related to failure to maintain fire safety components, electrical components, and plumbing systems in a safe and operating condition. Specific issues included dusty return grills in the dining room, a non-illuminating exit light in the SCU activity area, and removal of the ice machine cover due to freezing lines causing a safety hazard.

Visit Reason
The Adult Care Licensure Section and the Rutherford County Department of Social Services conducted an initial survey on 02/27/24 and 02/28/24.

Findings
The facility failed to ensure therapeutic diets were served as ordered for 2 of 2 sampled residents who had physician orders for no added salt and mechanical soft diets with ground meat. Errors in diet orders and meal preparation were identified due to miscommunication and vacant staff positions responsible for processing diet orders.
