Visit Reason
The Minnesota Department of Health conducted a full survey on October 1, 2025, to evaluate and assess compliance with state licensing statutes for The Sanctuary at St Cloud assisted living facility.
Findings
The facility was found in substantial compliance but had violations related to food service preparation per Minnesota Food Code, fire protection and physical environment including fire door malfunctions, emergency lighting, exit door locking, and deficiencies in fire safety training for staff and residents.
