The complaint investigation was initiated due to allegations that the center failed to ensure a clean, safe, and homelike environment; failed to provide care as ordered by physicians; failed to ensure residents did not exceed the level of care; failed to administer medications according to physician orders; failed to provide adequate supervision; and failed to issue refunds timely according to contract. The investigation included observations, interviews, and record reviews conducted from November 17 through November 24, 2025.
