The Adult Care Licensure Section conducted an annual survey and complaint investigation from 09/18/2024 through 09/20/2024 at The Viridian facility.

The facility failed to ensure medications were administered as ordered for 3 of 5 sampled residents related to medications for tremors and anxiety, pain control, and infection treatment. Additionally, the electronic medication administration record (eMAR) was found inaccurate for 2 residents regarding medication administration and documentation.

Visit Reason
The visit was a Construction Section Biennial Survey conducted to ensure the facility meets the 2018 Edition of the North Carolina State Building Code for Institutional Unrestrained Occupancies, the 2005 Minimum Standards and Regulations for Homes for the Aged and Disabled, and applicable portions of the current Rules for Adult Care Home of Seven or More Beds.

Findings
Deficiencies were cited related to physical plant code compliance, including a courtyard exit gate swinging inwards instead of outwards as required, and multiple conduit penetrations in fire resistant rated ceilings that were not properly sealed, potentially allowing fire and smoke to spread.

The Adult Care Licensure Section conducted an initial survey of THE VIRIDIAN facility on 11/03/22 to 11/04/22 to assess compliance with health care regulations and medication administration standards.

Findings
The facility failed to ensure timely referral to a vascular specialist for wound care, failed to clarify medication orders leading to administration errors, failed to administer medications accurately including blood pressure and nerve pain medications, and failed to ensure self-administered medications were properly labeled and secured.
