Inspection Report Summary for The Windham
The California Department of Social Services Community Care Licensing Division conducted a series of visits to WINDHAM, THE, a 740‑type group home located at 1100 E Spruce Ave, Fresno, CA. Over the span of 2021 the facility was subject to three incident‑related case‑management visits and one required‑year inspection. The inspections were carried out by Licensing Program Analyst A. Walton and its supervisor Melinda Hoffmann, with the administrator Kelly Metz and various directors present. All visits were unannounced or conducted by telephone due to COVID‑19 precautions.
In each incident visit the same deficiency—failure to assist residents with self‑administered medications as required by California Code of Regulations §87465(a)(5)—was cited. On March 17 2021 an employee (S1) mistakenly gave a medication intended for Resident R2 to Resident R1, and on April 18 2021 a 10 mg dose of zolpidem was administered to R1 instead of the ordered 5 mg. A later incident on November 20 2021 saw Resident R1 receive 10 mg of oxycodone rather than the prescribed 5 mg. For each violation a civil penalty was imposed: a $250 fine for the March incident, a $1,000 fine for the April incident, and another $1,000 fine for the November event. A Plan of Correction was developed with the administrator after each visit, and the facility received written notices of its appeal rights. The required‑year inspection on June 2, 2021 found the facility free of deficiencies, noting adequate infection‑control practices, resident hygiene, medication storage, and a 30‑day medication supply.
The pattern that emerges is one of repeated medication‑administration errors involving the same employee and similar residents. The penalties and citations indicate a sustained compliance issue with resident safety protocols. While the facility has demonstrated compliance with broader infection‑control and facility‑management standards, it continues to face regulatory scrutiny for medication errors that pose immediate health risks to residents. Continued monitoring and robust corrective action will be essential for WINDHAM, THE to eliminate repeat violations and to safeguard the well‑being of its residents.
