Inspection Report Summary for Traditions Memory Care of Newton

The most recent inspection on December 11, 2025, found the facility in compliance based on acceptance of a plan of correction. Earlier inspections showed a pattern of deficiencies related mainly to care planning, resident safety, and abuse prevention, with substantiated complaints involving failure to develop effective care plans and inadequate supervision to prevent resident-to-resident altercations. Prior citations also included issues with mechanical lift use, immunizations, and oral hygiene assistance. Complaint investigations were mostly unsubstantiated except for the substantiated findings in October 2025 regarding abuse prevention and accident hazards. The facility appears to have made improvements following earlier more extensive deficiencies, with recent plans of correction accepted and certification maintained.
