The inspection was conducted as an annual State licensure survey combined with a complaint investigation at the facility on 11/21/2024.
Findings
The facility was found deficient in multiple areas including failure to complete six-month medication reviews for some residents, missing Ultimate User Agreements prior to medication administration, lack of physician orders and medication availability for several residents, improper labeling of over-the-counter medications, missing initial Activities of Daily Living assessments for some residents, and incomplete cultural competency training for certain employees.
