Visit Reason
The inspection was an unannounced full renewal inspection conducted to review the facility's compliance with licensing regulations.

Findings
The inspection identified multiple deficiencies including failure to post the previous inspection summary, lack of grab bars in several toilet areas, missing emergency evacuation diagrams on the fourth floor, incomplete medical evaluations for residents, inaccurate glucometer calibrations, incomplete medication administration records, failure to follow prescriber's orders for blood sugar checks, lack of resident education on medication refusal rights, incomplete preadmission screening forms, and incomplete initial resident assessments.

Licensing inspection summary for the renewal inspection conducted on 11/14/23 was not posted in a conspicuous and public place in the home.
No grab bar, hand rail or assist bar at multiple toilet stalls on the 2nd and 3rd floors and in room 301.
No emergency evacuation diagram posted on the fourth floor of the home.
Medical evaluation for Resident #1 was not completed within required time frames.
Medical evaluations for Residents #1 and #2 missing required information such as ability to self-administer medications, health status, mobility assessment, birthdate, height, weight, and medications.
Glucometers for Residents #1 and #4 were not calibrated correctly and blood glucose records did not match glucometer readings.
Medication administration records (MAR) for Residents #1, #2, and #4 were not signed by staff to indicate medications were administered.
Resident #4's blood sugar checks were not taken at the ordered times as per physician's orders.
Residents #1 and #3 were not educated on their right to refuse medication if they believe there may be a medication error.
Resident #1's preadmission screening form was not completed within 30 days prior to admission.
Resident #2's initial assessment did not include the resident's need, use, and plan to protect from dangers of the enabler/assist bar attached to the bed.
