The California Department of Social Services Community Care Licensing (CCLD) conducted a series of evaluations for the residential care facility TRINITY HILLS ESTATES - WALNUT (Facility Number 198603643) over the course of more than a year. The initial inspection on 02/21/2023 was a COMP II interview with administrator Kenny Yu, who confirmed understanding of Title 22 regulations, licensing requirements, and admission policies. No residents were present at the time, and the facility’s census was zero. The pre‑licensing visit on 03/14/2023, led by Licensing Program Analyst Tao, verified that the single‑family home at 617 Walnut Ave met structural and safety standards, held a fire clearance and a Dementia Care plan, and was prepared for up to six residents. Staff and residents’ records were noted to be stored in locked cabinets, and the facility demonstrated compliance with fire, ventilation, and sanitation requirements.
An unannounced annual inspection on 04/18/2024 found the facility operating with its full census of six residents, including two on hospice under an approved waiver. The site remained compliant with most Title 22 provisions: grab bars, nonskid surfaces, proper temperature controls, functional alarms, and accessible food and medication storage. However, the inspection identified a serious deficiency under CCR 87465(h)(6). Records of centrally stored prescription medications for resident #1 were incomplete and did not match the medication log, with multiple days of discrepancy for several drugs. The licensee had not provided an explanation, creating a potential health‑safety risk. A Plan of Correction was required, with a due date of 04/19/2024.
Throughout the review cycle, the facility consistently met structural and safety standards, maintained proper staffing documentation, and adhered to licensing procedures. The primary concern that emerged was the lapse in medication record‑keeping, a critical component for resident safety and regulatory compliance. The CCLD documentation reflects that the facility was notified of the deficiency and has been given a brief window to submit corrective measures before further action.
