Visit Reason
The Minnesota Department of Health conducted a full survey to evaluate and assess compliance with state licensing statutes for an assisted living facility.
Findings
The facility was found to be in substantial compliance overall but had several level two violations including failure to ensure an individual abuse prevention plan included specific measures, non-functioning and non-interconnected smoke alarms, poor physical environment maintenance, incomplete fire safety and evacuation plans, lack of resident fire safety training, insufficient employee evacuation drills, and limiting resident rights by prohibiting alcohol and drug use on premises.
