An unannounced onsite Verification Visit was conducted as a follow-up to the 2nd Initial and Additional Space survey completed in December 2024 at Victory Visions Community Connections Day Center.
Findings
Victory Visions was found to be in compliance with Wisconsin Administrative Code DHS 105.14 regulations for Adult Day Care Centers. Two participant files were reviewed during the survey.

An unannounced onsite 2nd Initial and Additional Space survey was conducted to assess compliance with Wisconsin Administrative Code DHS 105.14 regulations for Adult Day Care Centers and to evaluate additional space requested by the facility.
Findings
The facility was found out of compliance with multiple regulations including failure to screen participants for communicable diseases, improper medication labeling and documentation, failure to document meals served, lack of hand-washing facilities in the kitchen, improper food storage and labeling, and poor maintenance of the new additional space with safety hazards.

An announced onsite initial certification survey was conducted from 09/26/2023 through 09/28/2023 at Victory Visions Community Connections Day Center in Waterloo, WI to assess compliance with Wisconsin Administrative Code DHS: 105.14 for Adult Day Centers.
Findings
The facility was found out of compliance with multiple regulations including failure to post accurate patient rights information, lack of safe medication storage, absence of transportation schedules, missing thermometers in refrigerators/freezers, unsafe ramp identification, no telephone on premises, reversed bathroom sink faucet labels, improper hot water temperature, missing emergency evacuation plan, and absence of smoke detectors in activity rooms and hallways.
