The California Department of Social Services repeatedly inspected VINE RIDGE AT CLOVERDALE in late 2021 and throughout 2022. Residents and staff wore masks in common areas, and the facility’s Activities Director facilitated daily programming. No deficiencies were cited. However, the report highlighted that caregivers had not yet undergone N‑95 fit‑testing, an issue the administrator was actively pursuing to meet CalOsha standards.
The most critical concerns arose in 2022. On 3/11/2022 a Case Management visit uncovered an elopement incident in the Memory Care unit. Two staff members were preoccupied with other residents and did not respond to the delayed egress alarm, allowing a resident to wander into the neighborhood. This event triggered a deficiency under the Care of Persons with Dementia regulations and required an immediate Plan of Correction. Subsequent visits on 8/19/2022 and 8/30/2022 investigated complaints about delayed egress doors and exterior gates that failed to alarm and were improperly latching. The facility corrected the doors and gates, repaired the delayed egress mechanism, and updated fire clearance documentation. No further deficiencies were cited after these corrective actions.
