On 09/17/2025, the Bureau of Assisted Living, Southern Regional Office, conducted a complaint investigation and abbreviated licensing survey at Vista Pointe, a community-based residential facility in Menomonee Falls, WI.

As a result of the survey, 5 deficiencies were identified. The complaint was unsubstantiated.
